Community Care Assistant – Criminal Record Disclosure Form

CCA 4 - Criminal Record Disclosure

Due to the nature of the work for which you have applied this post requires that all convictions and cautions must be declared, and that no conviction can be deemed spent, in line with the Rehabilitation of Offenders Order (NI) 1978 and the Rehabilitation of Offenders (Exceptions) Order (NI) 1979.

Therefore, please complete the following questions:

Do you have the right to work in the UK without restrictions?
Have you ever been convicted of a criminal offence?
Are you aware of any prosecutions outstanding or any pending court action against you?
Are you currently subject to any criminal investigation?
Is there any reason why you cannot work in a regulated activity?

I understand that if an offer of employment is made, it is subject to an Access NI Enhanced Disclosure.  Access NI enables organisations in Northern Ireland to make a more informed recruitment decision by providing criminal history information about anyone seeking paid or unpaid work in certain detained areas such as working with children or vulnerable adults.

I understand that I am required to meet the cost of the certificate, £33.

Please confirm you consent for an Access NI check to be completed and are willing to provide relevant payment
